Remote Virtual Supplemental Case Management Coordinator
Your Role as a Virtual Supplemental Case Management Coordinator
This remote role plays a crucial part in our Preferred Communities (PC) Intensive Case Management (ICM) Program. As a Virtual Supplemental Case Management Coordinator, you’ll provide strengths-based, trauma-informed, and client-centered remote case management to individuals who have exited initial resettlement services but still require ongoing support. Furthermore, you will assist highly vulnerable groups, including single-headed households, survivors of domestic and gender-based violence, older refugees, unaccompanied youth, individuals facing mental health challenges, those with disabilities, unanticipated arrivals, and LGBTQ+ refugees.
You will work closely with clients, colleagues, and external partners. Consequently, you’ll develop personalized service plans, coordinate resources, and guide clients through various U.S. systems. This encompasses helping with financial assistance, healthcare navigation, transportation, and more. Your expertise as a remote case manager will be invaluable.
Key Responsibilities of a VSCM Coordinator
Your main responsibilities will include, but are not limited to:
- You will provide remote client support and crisis intervention.
- You will develop individualized, SMART Self-sufficiency plans.
- You will communicate proficiently and effectively with clients, thereby demonstrating knowledge and digital literacy.
- You will lead or co-lead efforts to coordinate services and service planning with other providers and agencies.
- You will collaborate with the VSCM team, IRC colleagues, and community members. This ensures you identify and develop partnerships to connect clients to nearby resources that meet client needs.
- You will educate and guide clients in problem-solving strategies and navigating various U.S. systems, simultaneously developing a supportive relationship.
- You will provide clients with extended orientation on a variety of topics, such as health, navigating transportation systems, and navigating physical healthcare systems at local and state level(s).
- You will assist clients with requesting financial assistance when applicable. Moreover, you will track the received financial assistance within case files.
- You will maintain up-to-date documentation in the client’s case file to ensure services are properly documented throughout the client’s service period. Additionally, you will document all services provided in IRC’s database, Efforts to Outcomes (ETO).
- You will assess and make client referrals for other services as appropriate.
- You will perform other duties as assigned.
What You’ll Bring to this Case Management Role
We are looking for candidates who possess:
- Equivalent work experience or a Bachelor’s degree in Social Work or a related field of study (preferred).
- A minimum of 2 years of case management experience that includes working with refugee and/or immigrant populations.
- Demonstrated knowledge of U.S. Refugee Resettlement and programs.
- Strong relationship-building skills and the ability to effectively build internal and external relationships.
- The ability to successfully work and communicate effectively in varied global environments and contexts.
- The ability to work independently and collaboratively in a remote setting.
- The ability to communicate proficiently and effectively with clients.
- As a self-starter, you should have strong problem-solving and organization skills, plus the ability to prioritize and manage time effectively.
- Fluency in English, both spoken and written, is required. Proficiency in language(s) spoken by client group is desired; preferably proficiency in Dari and/or Spanish.
- High proficiency in Microsoft Office applications (Word, Excel, Outlook). Additionally, you should have the ability to use the internet and other digital tools for data entry, research, and problem-solving purposes.
- The ability to work through change in a proactive and positive manner.
