To support this important mission, a Remote Care Coordinator plays a key part from day one.


Your Daily Responsibilities

As a Remote Care Coordinator at Frontier Psychiatry, you become an important member of the operations team, managing many administrative tasks related to patient referral management and scheduling. This position holds a crucial role in making it easier for patients to get quality psychiatric and addiction care in underserved communities. Beyond simply managing referrals and clinician schedules, the Remote Care Coordinator also does important reporting work and takes on other projects as supervisors request.

Specifically, a successful professional in this role will:

  • Efficiently process all incoming referral faxes within three business days.
  • Proactively follow up on pending referrals, making at least three contact attempts to patients and ensuring the referral loop closes by telling referring providers.
  • Skillfully manage patient referrals, including clearly talking about details and expectations with patients.
  • Routinely verify insurance eligibility for telehealth services, confirming validity and active coverage.
  • Maintain clear communication with referring providers throughout the entire referral process.
  • Accurately enter complete patient information and insurance details into the Electronic Health Record (EHR).
  • Register patients by getting information from incoming referral documents, using all available resources to get more details when needed.
  • Tell the Lead Referral & Scheduling Care Coordinator about any new referring facilities or providers that need to be added to the EHR.

Scheduling & Patient Coordination

  • Expertly schedule new patient appointments, follow-ups, and other needed pre-visit sessions.
  • Promptly answer phone calls, process voicemails, manage patient portal messages, and handle all inquiries quickly.
  • Make sure provider schedules are fully used, actively helping with follow-up scheduling needs.
  • Manage cancellations, reschedules, and no-shows, and swiftly correct registration errors as assigned.
  • Assist with completing tasks emailed to the scheduling group.
  • Maintain accurate patient records within the EHR system.
  • Collect copayments and/or outstanding balances when asked, and provide patients with cost estimates for services according to policy.
  • Promptly correct registration errors as assigned by the billing department.
  • Fulfill all daily assigned duties promptly and efficiently.
