Intensive Case Management Caseworker – Salt Lake City, Utah

Specifically, this full-time, in-person position directly addresses significant barriers to safety and wellness for clients across the United States. You’ll provide strengths-based, client-centered support to diverse individuals. These include single-headed refugee households, survivors of domestic/gender-based violence, older refugees, youth without permanent guardians, and those experiencing mental health or physical difficulties. Furthermore, you’ll assist unanticipated refugee arrivals, secondary migrants, and refugees who identify as LGBTQ+. This vital caseworker role offers compensation between USD 23.75 – 26.49 Hourly.
Key Responsibilities for an Intensive Case Management Caseworker
As an Intensive Case Management Caseworker, you’ll handle a range of critical duties. Initially, you’ll determine client eligibility for ICM programming. Following this, you’ll conduct intake assessments, review program expectations, and explain confidentiality. Next, you’ll develop individualized, SMART Self-sufficiency plans for each client enrolled in the ICM program. Subsequently, you’ll evaluate client progress every six months, identifying new needs and actions required to meet their goals.
Additionally, you’ll assist clients in navigating healthcare and social service systems. This includes helping them apply for benefits, and scheduling and attending appointments. Moreover, you’ll conduct community outreach. You’ll maintain a strong network of service providers meeting client needs. Indeed, this refugee support specialist will also maintain robust relationships with local and state-level refugee and health coordinators, ensuring comprehensive support for clients.
Furthermore, you’ll provide clients with extended orientation. Topics range from women’s health to navigating transportation and physical healthcare systems. You’ll also respond to client crises, with supervisor guidance as needed. Beyond that, you’ll assist clients with financial assistance requests when applicable. You’ll diligently track all received financial assistance within case files. Maintaining up-to-date documentation in client case files is essential. You’ll document all services in IRC’s database, Efforts to Outcomes (ETO). Finally, you may train and lead volunteers and interns in this caseworker role.

This caseworker role combines a standard office environment, some remote work, and ‘field’ time within the service delivery area to perform the above outlined responsibilities. Therefore, occasional weekend and/or evening work may be necessary.
